The issue of prevention and treatment of pressure ulcers among the geriatric patients

Abstract: This bachelor thesis deals with the issue of prevention and treatment of pressure ulcers among the geriatric patients. Aims of the thesis: Find out which interventions nurses are practicing to prevent pressure ulcers among the geriatric patients. Explore the nurses' experience in treating pressure ulcers among the geriatric patients. Methodology and research set: To achieve the objectives, research questions were created. What are the interventions used to prevent pressure ulcers among the geriatric patients? What is the experience of nurses in treating pressure ulcers in the selected institutes? The empirical part of this bachelor´s thesis was created using a qualitative research method. The data was collected using semi-structured interviews which were conducted with nurses at selected departments of three hospitals in the South Bohemian Region. The research sample "nurses" consists of 15 nurses in total, each with a minimal experience of 3 years. The interviews were complemented by a long-term direct, participant observation involving 5 nurses from one of the three South Bohemian hospitals in the selected department. The data obtained were processed using the pen and paper interviewing method (PAPI). Open coding was used and the data were then sorted into eight categories and 15 subcategories. Results: The results of the survey showed that the nurses encounter the issue of pressure ulcers among the geriatric patients almost on daily basis. Nurses were able to list the interventions used in the prevention of pressure ulcers among the geriatric patients. Our investigation revealed that some nurses did not take the same action in practice when compared to their statements and described their practices in treating pressure ulcers among geriatric patients only in theory. They are knowledgeable about treating pressure ulcers patients but in practice they do not abide by certain principles. These include the use of protective equipment during bandaging, adherence to indications, communication with the patient, monitoring the patient's general condition, maintaining sterility, supervision and care of the area around pressure ulcers. Recommendations for practice: We recommend the inclusion of more courses about the topic of pressure ulcers in nurse education as it is essential to have sufficient information and regular education to provide quality care. We also recommend that in addition to theoretical knowledge nurses should be given practical training in the prevention and treatment of pressure ulcers.
Pressure ulcer prevention strategies

Aims: The aim of the bachelor thesis was to determine the level of knowledge of general nurses in the field of prevention and treatment of pressure ulcers. Methods: The target group of respondents were general nurses employed at the Intensive Care Unit of the Orthopaedic and Traumatology Clinic of the University Hospital Kralovske Vinohrady in Prague. There were 21 respondents in total. Results: The overall success rate - knowledge of the monitored criteria was 62.33 %. The results of the actual survey are presented in clear bar charts. Comparsion of the approach in the prevention of pressure ulcers in the departments of surgical and non-surgical fields

The aim of this bachelor thesis is to map and subsequently compare the approach in the prevention of pressure ulcers in the departments of surgical and non-surgical fields of health facilities. Today, pressure ulcers are still among the most common chronic wounds, and patients at risk of developing them occur across all wards. The empirical part of the bachelor thesis has been processed by the method of qualitative research. The semi-structured interview technique was chosen for data collection. Interviews were conducted with nurses working in the departments of surgical and non-surgical fields in two South Bohemian hospitals. After processing the obtained data, the results of the research survey were divided into 3 categories, which were further divided into subcategories. The results of the thesis not only provided information on the differences in the approach in the prevention of pressure ulcers among departments of surgical and nonsurgical fields, but a fundamental difference also emerged in the comparison of departments of surgical fields between the monitored hospitals. The differences found concerned the assessment of the risk of pressure ulcers before surgery and the application of preventive measures to prevent pressure ulcers during the perioperative period.The results of the thesis have shown that nurses from the department of nonsurgical fields are much more actively educated in the issue of pressure ulcers, which is also relevant in the departments of surgical fields. Furthermore, it has been found that nurses from the department of surgical specialties, in contrast to nurses from the department of non-surgical specialties, have a deficit of knowledge in positioning patients with hemiparesis or hemiplegia. However, even these patients are commonly encountered by nurses in the operating wards and should therefore be able to provide them with quality care in the prevention of pressure ulcers. The achieved results of qualitative research can serve as an incentive to improve the management of pressure ulcer prevention in health care facilities and thus increase the quality of nursing care provided.
Monitoring of Pressure Ulcer Care in the Internal Department.

The topic of the present bachelor thesis is the monitoring of pressure ulcer care in an internal ward. Pressure ulcers are a significant problem area on many wards and therefore an aspect of patient care that should be given sufficient attention. The first aim of the present study is to map the knowledge of non-medical healthcare professionals in the prevention of pressure ulcers in selected internal wards. The second objective is to map the knowledge of non-medical health workers in the area of pressure ulcer prevention in selected internal wards. The third objective is to map the knowledge of non-medical health care workers in the management of pressure ulcers in selected internal wards. To achieve the objectives, quantitative research in the form of interviewing through a non-standardized anonymous questionnaire was used. The research survey was conducted in three internal wards and three aftercare wards in a South Bohemian hospital in March 2022. A total of 35 closed-ended questions were asked to the respondents. For the research population, 110 questionnaires were distributed with a return rate of 74,5 % (n = 82). Thus, the research population consisted of a total of 82 (100,0 %) male and female non-medical health care workers from the internal wards (n = 41) and the aftercare wards (n = 41). The results showed that there was no statistically significant difference in the knowledge of pressure ulcer prevention between nurses from internal wards and those from aftercare wards. Also, the data confirmed that there was no significant difference in knowledge regarding pressure ulcer prevention between nurses from internal wards and those from aftercare wards. Similarly, there was no statistically significant difference in knowledge regarding pressure ulcer care between nurses from internal wards and those from aftercare wards. No one scored less than half of the possible points in pressure ulcer care. This indicates to some extent that knowledge in the areas studied is at a relatively good level in all wards.
Options of electrical stimulation of paretic muscles in patients withh spinal cord injury

Electrostimulation of neuro-muscular tissue is one of the elementary treating methods in physiotherapy. One of the areas of its use is rehabilitation of patients with spinal cord injury. As there doesnt exist enough resources written in czech language, I settled down to the goal of present the overview and options of its use in those patients. As completing this text, I came out of my knowledges drawn through the studies on 2nd Faculty of Medicine Charles University, specialized publications available in czech language and not least from specialized articles, case studies and literature of worldwide authors, because those are majority of informations aviable about this problematics. This work stays from parts about spinal cord with its injuries and elecrtostimulation and its practical use. Big portion of the work deals with usage of electrostimulation to improve the quality of life of spinal patients. Especially in prevention of pressure ulcers, respiratory diseases and osteoporosis. Last chapters are specially devouted to functional electrostimulation of upper and lower limb.
Evidence-based practice in the prevention and treatment of pressure ulcers in intensive care patients

Pressure ulcers are one of the quality indicators of nursing care, their presence reduces the quality of life of the patient, leads to a prolongation of hospitalization, increases costs of care and affects the budget of a health care facility. European Pressure Ulcer Advisory Panel or the European Advisory Board on issues of bedsores (EPUAP) leads and supports all European countries in their efforts to prevent and treat pressure sores. EPUAP working group has developed guidelines for the prevention and treatment of pressure ulcers. One of the important factors in providing the highest quality of nursing care is the ability of nurses to practically use the research results in the nursing process. This thesis deals with the effective use of the latest advances in the prevention of pressure sores and their treatment in the intensive care in Czech Republic. The main objective of this thesis is to explore the latest methods of prevention of pressure sores and care, according to Evidence-based practice principles EPUAP and analyze their practical implementation in selected intensive care wards. Nutritional interventions as a prevention and treatment of pressure ulcers in geriatric clients living in social services facilities Introduction: Pressure ulcers are chronic wounds caused by pressure and friction. This is localized area of cell damage caused by a microcirculation disorder. Prevention is essential especially for immobile clients who are easily creating and developing these chronic wounds. Clinical practice shows that client positioning, use of anti-decubital aids and good nutritional support is necessary. It is important to increase the supply of individual nutrients, especially proteins, and monitor their sufficiency for treatment and prevention. The theoretical part of this diploma thesis was devoted to the etiopathogenesis, classification, prevention and treatment of pressure ulcers especially in terms of nutrition. The empirical part was focused on the examination of the nutritional status of clients with pressure ulcers (using the Mini Nutritional Assessment-Short Form questionnaire) and the concretization of the nutritional needs and the real balance of the clients with newly established pressure ulcers at the beginning and after three months of nursing. Skin lesions prevention in perioperative care

Thesis objectives: The diploma thesis deals with the issue of prevention of skin lesions in perioperative care. In the theoretical part, the current state of knowledge is presented concerning the pressure ulcers identification due to the persisting real problems in differential diagnostics in clinical practice. Further, the thesis deals with the issue of wet and thermal lesions, which occur also during the perioperative care. In the empirical part of the diploma thesis, the first objective was to find the frequency of documented occurrence of decubital lesions in connection with the surgery in Jihlava Hospital for a period of one year. The second objective of the thesis was to find out whether, and what barriers exist when using specific preventive measures. The last aim of the diploma thesis was to compare the efficiency of preventive measures from the point of view of perioperative nurses and nurses from service units. Three hypotheses were formulated to meet the objectives of the thesis. Method for achieving the objectives: The set objectives were achieved through a retrospective analysis of the hospital information system data and a quantitative questionnaire survey. The study included nurses working in surgery, orthopaedics, traumatology, intensive care units and perioperative nurses in Jihlava Hospital. The total number of respondents was 140. Scientific benefits of the thesis: The research implies that the use of prophylactic aids in pre-operative preparation contributes significantly to reducing the risk of skin lesions. The addressed set of nurses did not indicate any serious barriers to their application and, on the contrary, considered it to be very effective. The results of the thesis can be used in practice, especially in a specific health facility for further care improvement. The findings and conclusions: On the research group we have verified that the declared use of preventive strategies in the area of prevention of lesions in perioperative care differs according to the type and focus of the workplace; (p = 0.000) the least frequent use was declared by the general nurse from orthopaedics, however, the methods used did not differ (p = 0,220). In addition, we found out that there was a statistically significant difference in the opinion on efficiency of prophylactic covering in lesion prevention (p = 0.001, the worst evaluation was by the respondents from orthopaedics) but only 2 respondents stated negative experience with their use.
